Insurance Information

Please Note: We take care of all of the billing with insurance companies and private payment options.

Advanced Surgical Mobile Eye Care Inc. accepts many insurance plans and the filing for payment. The following is a list of the insurances we accept:

• Medicaid – effective 01/01/10, Medicaid recipients are eligible up to 1 pair of glasses every 24 months and have eye exams as needed.
• Medicare – must have Part B to cover examinations
• Evercare – eligible for eye glasses, eye exams annually and medically needed
• Beechstreet
• First Health
• One Health Plan / Great-West
• Premier – examination only one time per year

HMO’s always need prior approval. The following list is of HMO insurances we accept only with prior authorization. We will obtain the authorization before treating the patient.
